Date Entered U.S.--If the student was born in the United States this date will be their birth date. If the student was bornoutside of the country the date should be equivalent of the Date Entered US School field.Date Entered US School--The US enter date should equal the students first day of school in this country. So unless this studentattended school outside the country and is coming back to you from outside the country then their enterdate is the date they started Kindergarten.Date Entered CO Public School--The Colorado enter date should equal the student’s uninterrupted start of public school in the state ofColorado. PreK and K do not count.Examples:1. 1st grade student starts attending from Texas. CO enter date is first day of school that you are registering for.2. 1st grade student starts attending from Denver. CO enter date is their 1st grade date.Description of Contact PreferencesContact Sequence - Adding a sequence number on contacts will prompt district staff to contact these persons in the order that you specify. Parent/Guardians should start with a sequence of 1 and Emergency Contacts should also start at a sequence of 1.
No Relationship - Marking this checkbox will indicate that this person does not share a relationship to the student. By checking this checkbox you are indicating that this person no longer has a relationship to the student. The relationship will be ended if one exists.
